Interventionist in the Florida Keys, Monroe County
Intervention in the Keys starts with a question no mainland family asks: how do we get them off the island?
From Key West to Key Largo, care is spread thin along one highway. We plan the conversation and the journey together, so a yes in Marathon or Key West turns into an admission instead of a long drive with second thoughts.
In most cities, the hard part of an intervention is the conversation. In the Keys, the conversation is half of it. The other half is distance. Detox and residential treatment are scarce on the islands, the public crisis unit sits in the middle of the chain, and the nearest large treatment market is at the top of the Overseas Highway, in Miami-Dade.
G3 is based in Austin, Texas, and travels to the Keys for the intervention itself. Everything before that, from the history to the rehearsals to the bed and the ride, happens by phone and video starting the day you call. By the time Matt lands, the route off the island is already set.
What makes the Keys different
A small community where everyone knows the bartender, the boat captain and the deputy. Seasonal work that hides heavy drinking until it doesn’t. One road north. Families who live far away and see their loved one twice a year. Each changes how we plan, and none of them means it cannot be done.
Down the Overseas Highway
Where help actually is, island by island.
Hospital and clerk
DePoo Hospital (Lower Keys Medical Center) appears on DCF’s list of Baker Act receiving facilities. The Clerk of Court sits at 500 Whitehead Street, 305-292-3458. Key West International Airport (EYW) is here.
The county crisis unit
Guidance/Care Center’s crisis stabilization unit is Monroe County’s public Baker Act receiving facility. The Clerk’s Marathon office is at 3117 Overseas Highway, 305-289-6027.
Plantation Key
The Clerk’s Upper Keys office is at 88770 Overseas Highway, Suite 2, 305-852-7145. From here, the mainland is the next stop north.
Most treatment
Residential programs, specialty psychiatric care and most detox capacity sit in Miami-Dade and beyond. Planning the trip there is the core of a Keys intervention.
Why the trip matters more than anywhere else we work
The Overseas Highway runs 113 miles from Key Largo to Key West and crosses 42 bridges, including the Seven Mile Bridge at Marathon, according to the Monroe County Tourist Development Council. It is beautiful, and for a family trying to move someone who agreed to treatment an hour ago, it is a very long time in a car. Every bar, every friend’s dock and every argument about turning around is on that road.
So we decide the route before the intervention, not after. When a flight is the cleaner answer, our escort travels with your loved one from Key West International Airport (EYW) to the program door. When the program is in Florida and a drive makes sense, a trained companion drives, not a worn-out parent. If your loved one needs medical detox first, we sequence it so there is no gap between a detox discharge and a residential bed. Our sober transport page explains how an escort works, and our guide to interstate intervention logistics covers the out-of-state version.
Timing is part of the plan too. The Keys depend on that one road, and storm season can close it or fill it with traffic heading north. When a hurricane is forecast, we would rather move the date than rush a fragile yes into an evacuation.
Baker Act and Marchman Act in Monroe County
File where your loved one is. In the Keys, that can be three different counters.
Monroe County is its own judicial circuit, the 16th (s. 26.021). A Marchman petition is filed with the clerk in the county where the person is located, and the clerk may not charge a fee (s. 397.681). The Monroe County Clerk keeps offices in Key West, Marathon and Plantation Key, each open weekdays 8:30 a.m. to 5:00 p.m. Call before you drive to confirm which one handles the filing.
Since the 2024 rewrite, an adult’s spouse, guardian, any relative, a service provider, or one adult with direct personal knowledge may petition for involuntary treatment (s. 397.68112). The hearing is set within 10 court working days (s. 397.68151), and an order can last up to 90 days (s. 397.697). An order is only as good as the bed waiting for it, and in the Keys that bed is usually on the mainland. Our Florida Marchman Act guide walks through the process.
For psychosis, mania or a suicidal crisis, the Baker Act allows an involuntary examination of up to 72 hours by ex parte court order, by law enforcement, or on a professional’s certificate (s. 394.463). Anything longer must be sought by a facility or treating provider (s. 394.467). In a 2020 funding request to the Legislature, Guidance/Care Center described its unit as Monroe’s public receiving facility with 11 emergency psychiatric beds and 8 emergency detox beds. That is the scale of acute care for the whole chain, which is why discharge planning matters so much here. Our Florida mental health intervention page explains how we plan around a short hold.
If someone has overdosed, is threatening suicide, or is in danger right now, call 911. On the islands, response can depend on where you are along the highway, so do not wait to see whether things settle.
Call or text 988 at any hour for the Suicide and Crisis Lifeline, answered in Florida by the 988 Florida Lifeline. Thriving Mind South Florida, the DCF managing entity for Miami-Dade and Monroe, lists the Monroe Mobile Response Team at 305-434-7660, option 8, available 24/7, and Helpline Inc. for the Florida Keys at 1-800-955-5504, also 24/7.
Monroe County recorded 19 drug poisoning deaths among residents in 2024, an age-adjusted rate of 18.5 per 100,000, according to FLHealthCHARTS. In a community this small, nineteen is a number most people can put names to.
Crisis help in the Keys
Choosing treatment from an island, without being sold to
Because so little treatment exists in the Keys, families here end up shopping the mainland, often South Florida, by phone and by search ad. That is exactly the market Florida’s Patient Brokering Act, s. 817.505, was written for: it makes it a felony to pay or accept any kickback, bonus or benefit for steering a patient. A free placement line is not automatically dishonest. It is fair to ask who funds it.
We check programs ourselves. Substance use providers must be licensed by DCF, searchable in the DCF provider search. Psychiatric facilities appear in AHCA’s FloridaHealthFinder. If step-down housing is part of the plan after treatment, we confirm the residence is certified through FARR under s. 397.487.
Coming home is its own question. Returning to the same island, the same job and the same friends after 30 days is hard anywhere and harder in a place this small. We help families plan aftercare that is realistic for the Keys, including whether a longer stay on the mainland makes sense. Our treatment navigation and guide to building an aftercare plan go further.
